As a patient of East Valley Dermatology Center and Valley Skin Cancer Surgery, you are able to obtain access to your medical information related to your visits. The patient portal facilitates better communication between you and your provider. It allows you to conveniently access certain information from the comfort and privacy of your home or office.
Information currently available through the portal:
- View upcoming and past appointments
- Update personal information
- View and update current medications
- Request medication refills
- Send and receive messages
Copies of medical records are currently not available on the portal. Please complete a Records Request Form and fax, mail or drop-off at the office for your provider to approve and release. Please allow 5-7 business days for records to be available.
To obtain a first time login:
Please contact the office at 480-821-8888 option 4 and speak with a staff member. You will need to provide a valid secure email address for us to activate your account. Once we obtain this information, you will receive an email with a temporary password. After you login for the first time, you will be asked to authenticate yourself and will be required to change your password to something unique that you will be able to remember.
