Policies

Appointments

New Patients: Please arrive 15 minutes before your appointment time to register at the front desk. We request that you print and complete the Registration Forms from our website and bring with you to your appointment to expedite the process.

Established Patients: Please arrive 10 minutes before your appointment time to update paperwork.
We do our best to see you in a timely manner. Therefore, if you are more than 10 minutes late, we reserve the right to reschedule you for later in the clinic session or another day. If you do not speak English, please make arrangements to bring a translator.

No-Show/Late Arrival: Office policy states that any patient that has more than 3 no-shows a year may be subject to discharge from the practice. Any patient that arrives over 10 minutes late for their appointment may be required to re-schedule their appointment for a later date. This missed appointment would count as a no-show on record.

Cancellations

Since we strive to provide such personal service to our clients, no-shows and cancellations impact us significantly; therefore we have a strict cancellation policy. If you are unable to keep your appointment, please call our office the day before to cancel or reschedule. A 24-hour notice is required. Thank you for understanding. There is a charge for less than 24 hour notice on many of our cosmetic appointments, please check with the front desk for more details.

Payments and Insurance

To ensure accuracy of your insurance on file, please present your insurance card at every visit. It is your responsibility for knowing and understanding your insurance benefits of the plan in which you have selected. East Valley Dermatology Center is not responsible for obtaining or explaining these benefits to you. If you have any questions, please contact your insurance plan prior to any services rendered. Be prepared to pay for your co-pay at the time of service. Although we will submit a claim to your insurance company on your behalf for contracted carriers, you will ultimately be responsible for any additional financial responsibility such as deductibles and co-insurance once the claim has processed. If you do not have your insurance card with you, you will be required to pay for the visit at the time services are rendered. We accept cash, checks, Visa, MasterCard, and Discover.

Payment is due at time of service for all self-pay and cosmetic transactions.

Note: We accept most major carriers, but some plans have individual networks that we are not contracted with. Please contact the office to confirm we accept your insurance. We do not accept the following insurances: AHCCCS plans; select Marketplace plans

Feel free to fax a copy of your insurance card (front and back) to us at 480-821-0888 before calling and we can provide the most accurate information based on your actual insurance plan.

After Hours Emergency

For emergent matters that cannot wait until regular office hours, please call 480-821-8888 and follow the prompts for the on-call phone in which you will leave a message regarding the urgent matter. Our on-call provider will return your call as soon as possible.

Prescription Refills

Please contact your Pharmacy for any prescription refill requests. After-hours, our on-call provider will try to accommodate your needs for medications that are needed on an emergent basis only.
If you obtain your prescription refills from our internal prescription desk at EVDC RX, please call 480-821-8888 and choose option 9, and our dispensary technician will assist you.
