Appointments:
Please arrive 15 minutes before your appointment time to register at the front desk. We do our best to see you in a timely manner. Therefore, if you are more than 10 minutes late, we reserve the right to reschedule you for later in the clinic session or another day. If you do not speak English, please make arrangements to bring a translator.
Cancellations:
Since we strive to provide such personal service to our clients, no-shows and cancellations impact us significantly; therefore we have a strict cancellation policy. If you are unable to keep your appointment, please call our office the day before to cancel or reschedule. A 24-hour notice is required. Thank you for understanding. There is a charge for less than 24 hour notice on many of our cosmetic appointments, please check with the front desk for more details.
Payments and Insurance:
To ensure accuracy of your insurance, present your insurance card at every visit. If you do not have your insurance card with you, you will be required to pay for the visit at the time services are rendered. This includes co-pays, deductible and co-insurance. We accept cash, checks, VISA, Mastercard, and Discover. Payment is due at time of service for all cash pay and cosmetic transactions.
After Hours Emergency
Call 480-821-8888 and follow the prompts for the on-call phone to leave a message on the voicemail system if you have a question outside of normal office hours. Drs. Geary and Chen are on call throughout the week and weekends for urgent matters.
Prescription Refills
Please contact your Pharmacy for any prescription refill requests. After-hours, Drs. Geary and Chen will try to accommodate your needs for medications that are needed on an emergent basis.
Note: We do not accept the following insurances: Pacificare, Tricare Prime, and all Ahcccs plans.
